1. Adult Application (anyone above the age of 16 years old) required forms: This is for passport and EMERGENCY TRAVEL CERTIFICATE – ETC.
- a) DHA-9 (a finger prints forms used to verify the applicant on the National Registry).This form is not downloadable. It is written
“ First Application for ID” but is also used for passport and Emergency Travel Certificate – ETC application for those applying outside the borders of RSA. - b) DHA – 529 and
- c) DHA-73
- d) Four (4) pictures size 2” x 2” colour. No teeth showing and No glasses.
- e) Passport fee is: $33-00 payment by Money Order and made Payable to SA EMBASSY. Unless the applicant will be at the Embassy in person then cash can be used (exact change).
- f) ETC fee is: $12- 00 payment by Money Order and made Payable to SA EMBASSY. Unless the applicant will be at the Embassy in person then cash can be used (exact change).
2. Children passport application:
- a) DHA -73 the most important area is to fill Section
- (C) Consent Section. Both parents MUST the form.
- b) DHA – 529
3. Changing Lastname/Surname due to Marriage:
Send a formal written request for the change of the last name…..

Please send a prepaid A4 (11” X8, 5”) size (return envelope with stamps) to the Embassy with a note stating that you want passport application forms for an adult to be sent to your address.
